How to apply for a passport for your child? While the steps to applying for a passport are simple, don’t wait until the last minute. If issued when the child is 15 years or younger, the passport is only valid for up to 5 years. Whether you are applying for a passport for the first time or need a new one, we’ve listed what you need to know.

Step 1

First, you’ll have to fill out the DS-11 form. Whether you are applying for a new passport or renewing, if the child is under 16, you must complete this form. Once finished, DO NOT SIGN. You’ll have to sign while you are with the acceptance agent during your appointment.

Step 2

Second, book your appointment with an acceptance facility. You must apply for your child’s passport in person. You can find a facility near you at iafdb.travel.state.gov

Step 3

Third, gather your documents. You need evidence of your child’s U.S. citizenship – an original birth certificate. You’ll also need to show your parental relationship, either the birth certificate or an adoption/divorce/custody decree.

Include a valid ID and if there is another legal guardian/parent, have their ID. Make copies of the birth certificate and the IDs.

You’ll also need two separate payments. The application fee is $100, and the execution fee is $65. If you need expeditated services, it is an additional fee. Acceptable forms of payment: personal check or money order. Credit/debit cards and cash are not accepted.

Step 4

Fourth, you need a valid passport photo. Some acceptance facilities will take the photo at the appointment for an additional fee. Or you can visit the local CVS or Walgreens for a professional passport photo that meets the requirements.

What to Bring to Child’s Passport Appointment:

  • Unsigned, completed DS-11 form
  • Photo
  • Original birth certificate (proof of US citizenship)
  • Parental consent or divorce/custody decree
  • IDs (both parents, if applicable)
  • Copies of all documents submitted

 

Your documents will be submitted after your appointment. It takes 8 to 11 weeks for everything to be processed and receive their passport. If you did an expedited service ($60 additional fee), then the passport should arrive within 2 to 3 weeks.
